Social Services

The Social Services/Discharge Planning Department insures that each patient admitted to the hospital has a planned program for needed continuity of care and/or follow-up at discharge, and is staffed with professionals who are trained to offer support, psychosocial evaluation and make recommendations for ways to deal with family crisis in a positive way.

Our Social Workers can assist you and your family in…

• Referral services ( To Hospice, Rehabilitation, Physical Therapy...)
• Nursing home placement
• Advance directives
• Home health services
• Medical equipment for home, ETC...

The Department offers services Monday through Friday, from 7:30 AM to 4:00 PM.
