Financial Assistance Program

Abbeville General Hospital will offer help to those who cannot afford their prescription medications starting January 2008. Starting after the first of the year, on Tuesdays Abbeville General Hospital will have a Financial Aid Coordinator in our Abbeville General Hospital Clinic (Rural Health Clinic) located at 2419 Alonzo St, Abbeville, LA and on Thursdays at our Maurice Community Care Clinic (Rural Health Clinic) located at 207 Milton Rd, Maurice, LA.

Our mission is to reach each and every person in Vermilion Parish and surrounding parishes who are struggling to pay for their medication with a message of hope.

Our Financial Aid Coordinator will also be available on these days for Financial Assistance offering assistance to uninsured patients to give them access to medical care at a sliding fee discounted rate.

To apply for Sliding Fee Scale (also known as Uncompensated Care) please click on the link below, print and fill out the application and return it to the address listed on the application or return it in person to the Financial Assistance Office with the required documents.

Yearly membership fee=$30.00 must be paid that day

Previous month’s proof of income. Example-paycheck stubs, letter from employer stating gross monthly income, copy of social security income letter or check or rental income receipts.

Previous year’s Federal income tax returns (1040’s).
